STEP 1: DOWNLOAD FORMS - After making this purchase*, you will receive an email confirming the purchase which contains a link for you to download forms -OR- you may sign in to your registered account on our website and download the documents from the ORDERS page. These forms are: New Patient, Health Assessment Questionnaire, Bio-Toxicity Questionnaire, Daily Diet Report - 1 week. Those documents need to be completed in full and returned to us. Include any blood work or health tests within the last 6-12 months.
STEP 2: RETURN FORMS - Please email your completed documents to customercare@druglessdoctor.com prior to the Wellness Consultation.
STEP 3: SCHEDULE CONSULTATION - Once we receive your completed documentation, you will be contacted to schedule a convenient time for Dr. Bob to call you and discuss his findings and recommendations.
Please Note: Once you have purchased a consultation, you must schedule within the first six months. After 6 months, if the consultation is NOT scheduled, it is considered expired and you will need to re-purchase. *Consultation purchases are NON-REFUNDABLE.
